Accident investigations are intended to determine the cause of an accident, to identify unsafe conditions or acts and to recommend corrective actions so that similar incidents don't occur in the future. The purpose is prevention, not blame. MTA Safety Consultants will conduct the investigation. This webinar discusses the MTA investigation process and how they are conducted. In the event that a consultant is unable to be there is a timely matter, managers and supervisors may need to conduct the investigation.
